Winger Chadwick, on loan to City from Stoke with a view to a permanent move in January, scored on his debut in the 3-1 defeat by Ipswich but was injured 10 minutes from time when he crashed into an advertising hoarding after a tackle by Town's Matthew Bates.
The 26-year-old was carried off and taken to hospital. His left shoulder took the brunt of the impact, but there was more concern at a gash on his knee and he underwent a clean-up operation on Sunday night.
City boss Peter Grant and physio Neal Reynolds are waiting for the surgeon's report before assessing how long Chadwick may be sidelined. The shoulder injury is believed to be less serious.
Midfielder Youssef Safri was the other weekend casualty, suffering from flu.
